Semaglutide is a GLP-1 (glucagon-like peptide-1) receptor agonist — a single-target molecule. Tirzepatide adds a second mechanism: it is a dual GIP/GLP-1 receptor agonist. Published head-to-head trial data (SURPASS program) reported greater changes in metabolic endpoints than GLP-1 agonism alone, which is why dual agonism became one of the most active areas in the field.
Retatrutide goes a step further as a triple agonist — GLP-1, GIP and glucagon receptors. The added glucagon activity is associated in phase-2 data with effects on energy expenditure, not just intake pathways.
